New patient registration.
New patients: please download the registration form, fill it out, print and sign it, and bring it with you to your first visit. You can type into the form on a computer or fill it in by hand.
Bring to your first visit
- The completed and signed registration form (2 pages)
- A photo ID
- All insurance cards, front and back
- A list of your current medications
- Your referral, if your insurance plan requires one
